Care Management

At Western North Carolina Community Health Services, our Care Management program is designed to address not just the medical needs of our patients but also the social determinants that affect their health. This includes providing support for challenges like food insecurity, lack of housing, or transportation, and personal safety concerns. We screen all our patients for these factors because we understand that health is influenced by a range of social and economic conditions. Our Care Management team provides health education on nutrition and physical activity, strategies for self-management of chronic conditions, and coordinates with external providers and agencies to enhance access to necessary care.
