Care Management

At Western North Carolina Community Health Services, our Care Management program is designed to address not just the medical needs of our patients but also the social determinants that affect their health. This includes providing support for challenges like food insecurity, lack of housing, or transportation, and personal safety concerns. We screen all our patients for these factors because we understand that health is influenced by a range of social and economic conditions. Our Care Management team provides health education on nutrition and physical activity, strategies for self-management of chronic conditions, and coordinates with external providers and agencies to enhance access to necessary care.

FREQUENTLY ASKED QUESTIONS

  • Eligibility for our Care Management services extends to any patient facing significant health risks, including those with chronic conditions linked to obesity, living with HIV, or having combined socioeconomic and mental health challenges. You may be eligible for different resources based on your diagnoses, insurance, demographics, etc.

  • WNCCHS Care Management actively integrates services to manage patients' health in a holistic way.

    Our team includes Registered Nurse Case Managers, Medical Social Workers, and Community Health Workers. Together, they work to develop personalized care plans especially for patients at risk of poor health outcomes due to complex medical or socioeconomic conditions.

    These plans are crafted in close collaboration with the entire health care team and the patient, ensuring that each plan is tailored to meet individual needs.

  • Patients with a Managed Medicaid plan may be eligible for services through the Healthy Opportunity Pilot (HOP) after an assessment is completed. Please ask your provider for an internal referral to Care Management to speak with a care manager about services. Learn more.

  • The Manna Foodbank Mobile Market hosted by WNCCHS is the fourth Wednesday of every month at 71 Fernihurst Drive, Asheville, 28803 from 11:00 a.m.-12:30 p.m. Remember to bring reusable bags! Learn more.

  • We assist with transportation to WNCCHS appointments, appointments to specialists, and community resources. Requests should be placed at least two business days prior to appointment to allow staff to connect with the appropriate transportation resource.
